Preoperative NUn Scores Predict GC Progression

There are few methods available to stratify oncological risks before surgery for gastric cancer (GC). Inflammatory markers like serum C-reactive protein (CRP) and albumin levels are linked to outcomes.

Masayuki Urabe and the team aimed to evaluate the associations between disease-specific mortality and 4 CRP-albumin-based indices (CAR, mGPS, OPS, and NUn score) in preoperatively assessed cases with resectable GC.

The survival outcomes of 1290 patients with GC who underwent oncological gastrectomy were examined. The predictive significance of preoperative CAR, mGPS, OPS, and NUn scores was evaluated using time-dependent receiver operating characteristic curves and Cox regression analyses.

The median follow-up period was 107 months. The preoperative NUn score demonstrated a notably superior area under the curve for predicting overall survival (OS) and disease-specific survival (DSS) compared to other parameters. Univariate Cox regression analysis revealed significant correlations between preoperative CAR, mGPS, OPS, and OS/DSS.

In multivariate Cox regression analysis, the preoperative NUn score, as a continuous variable, exhibited an independent association with OS (HR 1.32, 95% CI 1.16-1.50, per 1-unit increase, P< 0.001) and DSS (HR 1.23, 95% CI 1.02-1.49, P= 0.032). Conversely, the other 3 markers did not maintain independence for DSS.

The study showed that preoperative NUn scores consistently correlate with outcomes, including disease-specific mortality. This suggested their potential utility as a straightforward measure to assess the likelihood of systemic disease progression following meticulous surgery for GC, thus aiding in the identification of patients who may benefit from additional treatment modalities.
