HBV-Related HCC Shows Better OS Compared to Other Etiologies

Yi-Hao Yen and the team aimed to investigate the impact of chronic liver disease (CLD) etiology on the overall survival (OS) of patients diagnosed with hepatocellular carcinoma (HCC). Despite extensive research, the relationship between the etiology of CLD and OS in HCC patients remains inconclusive

Researchers performed an inclusive analysis inlcuding 3941 patients newly diagnosed with HCC at their institution between 2011 and 2020. In cases where patients presented with multiple CLDs, a hierarchical classification system was used, where hepatitis C virus (HCV) > hepatitis B virus (HBV) > alcohol a associated with alcohol > those without HCV, HBV, and alcohol use.

About 3941 patients were analyzed, revealing 1407 with HCV-related HCC, 1677 with HBV-related HCC, 145 with alcohol-related HCC, and 712 with all-negative HCC. Multivariate analysis indicated HBV as an independent predictor of mortality (HR: 0.856; 95% confidence interval: 0.745-0.983; P= 0.027). Patients with HBV-related HCC exhibited superior OS compared to other CLD etiologies (P<0.001).

Subgroup analyses showed significant differences in OS among various parameters including Barcelona Clinic Liver Cancer (BCLC) stages, 0-A (P<0.001); serum alpha-fetoprotein (AFP) levels≧20 ng/ml (P<0.001); AFP levels < 20 ng/ml (P<0.001); age > 65 years (P<0.001); and the use of curative treatments (P = 0.002). However, no significant differences were observed among patients aged ≤ 65 years (P = 0.304), with BCLC stages B-D (P = 0.973), or those undergoing non-curative treatments (P = 0.1).

The study concluded that patients with HBV-related HCC demonstrated superior OS compared to patients with other HCC etiologies.
