Health-Related Quality of Life (HRQoL) in long-term survivors post-radical cystectomy (RC) is a critical yet underexplored area of research for patients with bladder cancer, particularly when compared to the general healthy population (HP).
Riccardo Mastroianni and the team aimed to evaluate and compare the HRQoL characteristics of long-term survivors following RC with those of a healthy population control group.
Researchers looked at 2 groups: the “BCa cohort,” which included patients with high-grade bladder cancer who had RC and ileal Orthotopic Neobladder (iON) from 2010-2015, and the “HP cohort,” consisting of healthy people over 18 with no history of bladder or genitourinary cancers.
They matched these groups using a 1:1 propensity score matched (PSM) analysis and gathered HRQoL data through the European Organization for Research and Treatment of Cancer (EORTC) and generic QLQ-C30 questionnaires.
The results showed that 401 patients were enrolled in the study, with 99 in the BCa cohort and 302 in the HP cohort. After matching 67 patients from each group, self-reported HRQoL showed that the BCa cohort had better outcomes. Patients who had RC and iON reported significantly higher global health status (P<0.001), emotional (P=0.003), and cognitive functioning (P<0.001) compared to the HP cohort. They also experienced less fatigue (P=0.004), pain (P=0.004), dyspnea (P=0.02), and insomnia (P=0.005).
The study concluded that long-term survivors of RC and iON have significantly improved self-reported Health-Related Quality of Life compared to the healthy control group, reflecting greater awareness and better overall health status.
