ACE-27 Predicts Complications in Elderly AEOC

Mengna Zhao and the team spearheaded the study that aimed to assess whether Adult Comorbidity Evaluation 27(ACE-27) could predict perioperative outcomes and survival in elderly women with advanced epithelial ovarian cancer (AEOC) undergoing cytoreductive surgery.

Patients diagnosed with AEOC at the hospital between January 1, 2012, and January 1, 2021, were included. Exclusion criteria were patients under 65 years old or those with non-epithelial ovarian cancer. The retrospective application of ACE-27 evaluated comorbidities, categorizing patients into two groups that were low ACE-27 scores (none to mild) and high ACE-27 scores (moderate to severe).

About 222 elderly women diagnosed with AEOC were included, with 164 undergoing debulking surgery. Analysis revealed that patients with higher ACE-27 scores experienced Clavien-Dindo grade III + perioperative complications or unintended ICU admission more frequently (OR: 4.21, 95% CI: 1.28-14.35, P= 0.018).

Further subgroup analyses based on age < 70, BMI < 25 kg/m2, BMI, FIGO stage, and pathology indicated shorter overall survival (OS) for patients with severe comorbidities compared to those with none to moderate (HR: 3.25, 95% CI: 1.55-6.79, P= 0.002), as demonstrated by Kaplan-Meier survival curves using the log-rank test.

The study found that ACE-27 effectively predicts grade III + perioperative complications or unintended ICU admission and influences survival among elderly AEOC patients. This underscores its potential to identify high-risk individuals and guide treatment decisions.
