68Ga-FAP-2286 PET Imaging for Localized Bladder Cancer Shows Promising Results

In the clinical trial NCT04621435, patients (pts) with solid tumors received FAP-PET, including 16 pts with bladder cancer. Researchers reported 10 patients with clinically localized disease treated with curative intent at the time of imaging. Standard imaging with computed tomography (CT), magnetic resonance imaging (MRI), fluorodeoxyglucose-positron emission tomography (FDG-PET), or bone scan were conducted within 8 weeks of FAP-PET. The FAP-PET results recorded the maximum standardized uptake value (SUV) and size of FAP-PET-positive lesions. Positive lesions were defined as any activity area with at least 1.5 times higher SUV than the mediastinal blood pool.

Concordance of 12 FAP-PET scans was reported at 33% with standard imaging, while with FDG-PET it was 43%. FAP-PET showed a 100% concordance rate with true disease status in all 8 patients who had pathologic or radiographic follow-up. In two patients, FAP-PET was performed before and after neoadjuvant chemotherapy which revealed different results. One patient showed resolution of lymph node uptake, while the other had decreased but persistent uptake. Pathology from pelvic lymph node dissection in the latter patient indicated urothelial carcinoma.

The study concluded that FAP-PET can detect metastatic sites and identify falsely positive lymph nodes in cancers that are considered clinically localized. Additionally, it can improve response assessment to systemic therapy.
