A 74-year-old man presented to the Royal Columbian Hospital (New Westminster, BC, Canada) in December, 2023, with persistent unexplained fevers. Complete blood count revealed bicytopenia with a white blood cell count of 5·6 × 109/L (normal range 4·0–11·0 × 109/L), red blood cell count of 3·02 × 1012/L (3·5–5·0 × 1012/L), haemoglobin 70 g/L (135–180 g/L), mean corpuscular volume 87 fL (80–100 fL), and platelet count 74 × 109/L (150–400 × 109/L). A CT scan revealed splenomegaly (21 cm) but no hepatomegaly, masses, or lymphadenopathy.
